Asheville (Franklin/Macon County), North Carolina
Contract Duration: 13 Weeks (December 10, 2025 – March 11, 2026)
Estimated Weekly Pay: $1,898.64
Schedule: Day Shift | 5x8 Hours (8:00 AM – 5:00 PM) | 40 Hours Guaranteed

Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health setting serving the Franklin/Macon County area near Asheville, NC.

This role involves providing in-home rehabilitation services to patients recovering from illness, injury, or surgery. The Physical Therapist will focus on improving mobility, restoring independence, and delivering patient-centered care through individualized treatment plans.

The therapist will work independently in a field-based environment while collaborating with interdisciplinary teams to ensure optimal patient outcomes.

Work Setting: Field-based (patient homes)
Patient Volume: ~100 patients within branch
Charting System: HomeCare HomeBase (HCHB)
Dress Code: Business casual (therapy staff)

Therapists will provide care for:
Post-surgical recovery
Fall-related injuries
Mobility and functional impairments
Chronic condition management
Home health patients requiring rehabilitation services

Perform in-home patient evaluations and functional assessments
Develop individualized treatment plans based on patient needs
Deliver therapeutic exercises, mobility training, and rehabilitation interventions
Manage a caseload of approximately 5–6 patients per day
Monitor patient progress and adjust treatment plans accordingly
Educate patients and caregivers on home exercise programs and safety
Maintain accurate and timely clinical documentation (HCHB)
Coordinate care with interdisciplinary healthcare teams
Manage daily travel and scheduling efficiently

Minimum 1 year of Home Health Physical Therapist experience
Active North Carolina Physical Therapist License
Strong communication and documentation skills
Reliable personal vehicle for patient visits

Experience with HomeCare HomeBase (HCHB)
Previous travel experience
Ability to work independently in a field-based setting

40 hours guaranteed per week
Monday – Friday schedule
Shift: 8:00 AM – 5:00 PM
No weekend requirements mentioned

Coverage area: Franklin / Macon County region
Candidates must be comfortable working independently in a home health setting
Strong time management and organizational skills required
Reliable transportation is essential for patient visits
