Responsibilities
- Provide direct patient care to residents in a long-term care setting, including monitoring daily needs and comfort.
- Administer prescribed medications and treatments, document findings, and update care plans in the chart.
- Monitor vital signs, assess changes, and escalate to the supervising RN as indicated.
- Assist residents with activities of daily living, de-escalate agitation, promote safety, and maintain resident dignity.
- Collaborate with the interdisciplinary team to ensure continuity of care and adherence to infection control and safety protocols.
- Document accurately and complete, participate in care rounds, and respond promptly to changes in condition.

Requirements
- Active North Carolina LPN license or compact license
- BLS certification
- Minimum 1 year of geriatric LPN experience
- Local candidates only
- PCC experience preferred
- Ability to work 8-hour day shift (7:00 a.m. – 3:00 p.m.) in a long-term care environment
